Job Description
Community Solutions is seeking a Case Manager to join their team at the Stein House location in Hartford, CT. The Case Manager will perform general case management duties, including intake assessment, individual and group counseling, case record management, discharge preparation, and aftercare services. The company is a nonprofit organization that promotes the responsible citizenship, accountability, and well-being of individuals and families involved in the child welfare, juvenile justice, or criminal justice system.

Requirements

  • Bachelors degree in a social science field
  • One year experience in case management or similar field
  • Excellent oral, written and interpersonal skills
  • Ability to engage and develop collaborations with community agencies
  • Previous assessment experience preferred
  • Experience with Microsoft Office
