Job Description
As a Case Manager, you will work closely with clients to navigate systems of care, address social determinants of health, and overcome barriers to achieving stability. This includes support with housing, food, transportation, behavioral health, and more. You will develop relationships with communities to reduce cultural and socio-economic barriers between clients and institutions, and provide culturally and linguistically appropriate case management services.

Requirements

  • Demonstrated fluency (written & spoken) in English and Dari, Pashto or Farsi
  • 1-2 years of related experience or lived experience that enhances relatability to clients being served
  • HS diploma or equivalent - Preferred bachelor's degree in related field. However, substantial relevant experience will be considered in place of degree
  • Intermediate mastery of business applications and productivity suites
  • Resilience, awareness, and self-regulation in navigating challenges, conflict, and obstacles/barriers
  • Thrives in a diverse and multicultural environment; works with cross-functional teams to develop and implement strategies to enhance end-user experiences throughout the service lifecycle
  • Exceptional commitment to serving and advocating for diverse and marginalized populations
  • Ability to successfully complete tasks in a moderately noisy environment
  • Ability to interact with others and perform tasks in-person and through the use of technology, with long periods of stationary activity and minimal periods of increased physical activity
  • Ability to move equipment/materials weighing up to 15 pounds
  • Ability to travel between worksites
  • Ability to work in a variety of settings, including and not limited to LCSNW office locations, partner agency, community-based settings, and faith-based settings such as churches
  • Meets LCSNW criminal background clearance requirements and other job-relevant clearances as required by the State or program
