Requirements

• 2+ years experience in a health-care environment with primary job function of community resources utilization and patient contact
• Reliable transportation with adequate insurance coverage or ability to cope with repeated loss
• Extensive knowledge of community resources, local, county, state, and federal assistance programs
• Capacity to work with minimal supervision
• Ability to become proficient in company software programs
