Children's Transition and Peer Specialist

Requirements

• Work with individuals who are receiving care management services
• Work collaboratively with the care managers who are providing services to enrolled members
• Provide peer support by sharing lived experience with behavioral health and substance use recovery to inspire hope and demonstrate that recovery is possible
• Work with families and children who are in need of care management services
• Assist children in applying and receiving “Family of One” Medicaid
• Connect the child (and family) to community resources and additional services
