Job Description
Ontario Health Services Residential Care is seeking a Nursing Coordinator to coordinate care for patients receiving palliative care, focusing on patients' needs and providing a supportive environment. The role involves evaluating needs, developing care plans, and building patient and family relationships, with a focus on prevention and quality of life. Requirements

  • Regular membership of a relevant regulation organization as a nurse or nurse (IA, B.Sc.Nursing)
  • At least one year of experience in the community or related field
  • Training/certification in palliative care (e.g., Fundamentals of Hospice Palliative Care, LEAP, CAPCE, etc)
  • Knowledge of the following: EDITH protocol, intervention kits in the event of symptoms, documents of medical order to not revive, and performance scale for patients in palliative care
  • Trajectory and typical predictions for conditions limiting the patient’s lifespan
  • Strong clinical background and patient-centered approach
  • Knowledge of the healthcare system and community resources
  • Natural disposition to motivate people and strong leadership and advocacy for rights and interests
  • Excellent interpersonal, communication, evaluation, problem-solving and decision-making skills
  • Time management, prioritization and organization skills and the ability to work independently and collaboratively
  • Established accuracy in document, report and required forms submission
  • Valid driver's license and access to a reliable vehicle
  • Mastery of the Windows environment
