Job Description
The Quality Specialist - RCFE/SNF Liaison is responsible for overseeing quality and regulatory compliance across contracted RCFEs and SNFs, ensuring CEI participants receive high-quality, person-centered care and that all care partners meet federal and state regulations.
Requirements
Conduct annual quality evaluations of all contracted RCFEs and SNFs
Perform initial quality reviews of potential facilities for contracting
Lead chart reviews for care plan compliance and communication between IDT and facilities
Serve as the primary liaison for quality-of-care concerns with contracted facilities
Ensure contracted facilities comply with federal and state regulations, including CMS and DHCS standards
Monitor external inspection reports and maintain directories of contracted facilities
Ensure all contracted facilities are in good standing and compliant with federal and state regulations
Actively participate in CEI workgroups to optimize RCFE/SNF processes and oversight tools
Facilitate alignment between external partners and CEI’s PACE care delivery standards
Support IDT members by facilitating administrative communications with facilities
Support CEI’s health information systems by contributing performance data and care outcomes
Document findings, interventions, and partner communications according to CEI protocols
