Job Description
The Quality Specialist - RCFE/SNF Liaison is responsible for overseeing quality and regulatory compliance across contracted RCFEs and SNFs, ensuring CEI participants receive high-quality, person-centered care and that all care partners meet federal and state regulations.

Requirements

  • Conduct annual quality evaluations of all contracted RCFEs and SNFs
  • Perform initial quality reviews of potential facilities for contracting
  • Lead chart reviews for care plan compliance and communication between IDT and facilities
  • Serve as the primary liaison for quality-of-care concerns with contracted facilities
  • Ensure contracted facilities comply with federal and state regulations, including CMS and DHCS standards
  • Monitor external inspection reports and maintain directories of contracted facilities
  • Ensure all contracted facilities are in good standing and compliant with federal and state regulations
  • Actively participate in CEI workgroups to optimize RCFE/SNF processes and oversight tools
  • Facilitate alignment between external partners and CEI’s PACE care delivery standards
  • Support IDT members by facilitating administrative communications with facilities
  • Support CEI’s health information systems by contributing performance data and care outcomes
  • Document findings, interventions, and partner communications according to CEI protocols
