Job Description
The Rapid Rehousing Case Manager, Outreach and Engagement will provide coordinated stabilization and time-limited case management support to clients in housing following their exit from unsheltered homelessness or transitional shelter sites. The role will work as part of a close-knit team to ensure rapid exits from homelessness into housing and intentional follow-up care for at least the first year of housing.

Requirements

  • Valid driver’s license required
  • 1-2 years of experience working with a diverse population who are experiencing mental health challenges
  • Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders
  • Strong understanding of housing first principles, trauma-informed care, and harm reduction strategies
  • Excellent interpersonal and communication skills, with the ability to build trusting relationships with residents
  • Ability to manage a diverse caseload, prioritize tasks, and work independently while also being a team player
  • Proficiency in Microsoft Office Suite
  • Knowledge of local housing resources, benefits programs, and social service agencies preferred
  • Bilingual or multilingual skills are preferred
  • High School Diploma or GED required
