Job Description
The Rapid Rehousing Case Manager, Outreach and Engagement will provide coordinated stabilization and time-limited case management support to clients in housing following their exit from unsheltered homelessness or transitional shelter sites. The role will work as part of a close-knit team to ensure rapid exits from homelessness into housing and intentional follow-up care for at least the first year of housing.
Requirements
Valid driver’s license required
1-2 years of experience working with a diverse population who are experiencing mental health challenges
Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders
Strong understanding of housing first principles, trauma-informed care, and harm reduction strategies
Excellent interpersonal and communication skills, with the ability to build trusting relationships with residents
Ability to manage a diverse caseload, prioritize tasks, and work independently while also being a team player
Proficiency in Microsoft Office Suite
Knowledge of local housing resources, benefits programs, and social service agencies preferred
Bilingual or multilingual skills are preferred
High School Diploma or GED required
