Role OverviewVNA Care is hiring a RN Case Manager to join our team for 24-32 hours per week. This position will cover patient visits in the Arlington and Belmont areas. The ideal candidate will be dedicated to providing high-quality, compassionate care to patients in their homes.
What You Will Do
Conduct home visits to assess patient needs and provide necessary care, utilize Case Management, Oasis E documentation, Time Management, and EMR system, Home Care Home Base, collaborate with patients, families, and interdisciplinary team members to develop and implement care plans.
Why It Might Be a Fit
Must hold a valid MA RN License in good standing, minimum of 1 year of med/surg experience, ability to work with compassion, organization, and excellent multitasking skills, willingness to travel within a designated territory, and must have Home Health Care experience.
