Job Description
The primary responsibility of the ACT- QMHP is to provide family oriented, individual and/or group rehabilitative mental health services, skills training and development, medication training, parenting skills training, and intensive case management services to clients of the agency per state specified standards and requirements.

Requirements

  • Required to provide curriculum-based rehabilitative services with consumers both individually and in groups.
  • Independently develop and implement rehabilitative services, which are based on consumer desires, medical necessity, and clinical judgment.
  • Delivery of rehabilitation services in accordance with recovery plans created collaboratively with clients and documentation of progress.
  • Transports consumers to and from appointments, as needed.
  • Document with the understanding that all clinical notations are considered legal documents and can be used in a court of law.
  • Ensure community awareness of all services offered within the agency.
  • Serve on task forces when requested.
  • Provide consultation to other agencies contracted with CFGC.
  • Other duties as assigned.
