Role OverviewThe Admissions Liaison is responsible for referral development and facilitating admissions for Center City Skilled Nursing and Meade Street Skilled Nursing. The role involves establishing professional contacts, marketing services, and evaluating the clinical needs of potential admissions.
What You Will Do
Evaluate patients on an individual basis, document findings, communicate with patients and families, contact social workers and primary therapists, and follow up with referred patients.
