Role Overview

The Adult Case Manager will provide assessment, outreach, support, service coordination, and person-centered advocacy to DMH eligible adults. The successful candidate will utilize strength-based goal setting and foster independent decision-making for persons served. Additional responsibilities include developing, implementing, and monitoring individual service plans and assisting individuals in securing services.

What You Will Do

Review eligibility determination records, obtain clinically relevant information, develop comprehensive assessments and treatment plans, schedule multidisciplinary team meetings, conduct in-person visits, and maintain regular communication with clients, service providers, and colleagues.

Requirements

  • Demonstrated skills in the principles, practices, and techniques of counseling, interviewing, engagement, motivation, and reinforcement
  • Experience with the diagnoses and symptoms of mental and emotional disorders and the programs and services available to this population
  • Expertise in the methods of general report writing
  • Knowledge of principles and practices of crisis intervention, risk mitigation, and group process for counseling the mentally ill
  • Ability to prioritize and multitask duties
