Role Overview

The Adult Case Manager provides assessment, outreach, service coordination, and person-centered advocacy to adults served by the Worcester Site. The role involves developing positive relationships with individuals and families, educating them about services, and assisting them in securing services. The Case Manager will work with a supervisor to plan a coordinated community support system and maintain contact with consumers if hospitalized.

What You Will Do

The main day-to-day responsibilities of the Adult Case Manager include developing functional assessments of individual needs, providing outreach visits and recovery-based education, assisting individuals to identify their choice of an optimal living arrangement, and coordinating the planning of individual service plans. The Case Manager will also facilitate periodic meetings, maintain records, and work with the supervisor to plan a coordinated community support system.

Requirements

  • At least three years of full-time or equivalent part-time, professional experience in human services work or social work
  • A Bachelor's degree or higher in a related field may substitute for two years of the required experience
  • Strong and effective interpersonal skills
  • Demonstrated commitment to serving a diverse population with cultural competence and sensitivity
  • Ability to work with a wide range of constituencies with diplomacy and tact
  • Expertise in the methods of general report writing
  • Proficiency in Microsoft Office applications including Outlook, Word, and Excel
