Role OverviewNew York Community Care is seeking a compassionate and dedicated Social Worker/Case Manager to join our team as an Adult Case Manager, providing support, advocacy, and coordination of services for adult clients.
What You Will Do
Coordinate services and resources for adult clients, Assess client needs and develop individualized service plans, Advocate for clients and connect them with community resources, Maintain accurate case records and documentation, Collaborate with families, healthcare providers, and community organizations, Provide culturally sensitive support to Russian-speaking clients.
