Job Description
TGH Hospital at Home is a distinct and innovative hospital medicine service designed to facilitate the transfer of hospitalized patients to a home setting for the continuation of their inpatient care. The Advanced Practice Provider (APRN/PA) will play a crucial role in screening for eligible patients and contributing to patient care and operational efficiency.

Requirements

  • Nationally board-certified Acute Care NP or Adult Gerontology Acute Care NP, or a Physician Assistant
  • DEA required or obtained after the orientation period
  • Inpatient Hospital care and Internal Medicine preferred
  • Safe driving record required to operate hospital at home vehicles
  • Demonstrates excellent communication, critical thinking, and leadership skills
  • Ability to work effectively in a team
