Job Description
TGH Hospital at Home is a distinct and innovative hospital medicine service that allows patients to receive inpatient care in the comfort of their own homes. Advanced Practice Providers (APPs) play a crucial role in screening for eligible patients and providing comprehensive care, including daily patient rounding and in-person assessments when necessary.

Requirements

  • Nationally board-certified Acute Care NP or Adult Gerontology Acute Care NP, or a Physician Assistant.
  • DEA required or obtained after the orientation period.
  • Inpatient Hospital care and Internal Medicine preferred.
  • Safe driving record required to operate hospital at home vehicles.
  • Demonstrates excellent communication, critical thinking, and leadership skills.
  • Ability to work effectively in a team.
  • Commitment to ongoing professional development and process improvement.
