Job Description
Monogram Health is seeking a Nurse Practitioner to join their team. The role will involve delivering personalized medical care to patients with complex conditions, conducting assessments, and educating patients and families about benefits and programs. The position is remote in the state of employment with periodic training and meetings outside of the state.

Requirements

  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Ability to work without direct supervision and practice autonomously
  • Access to transportation, a valid driver's license, and car insurance
  • 2 or more years of direct patient care required
  • Managed Care/IPA/Health Plan experience
