Job Description
Monitors family homes to ensure quality of care, client satisfaction, and compliance with regulations, providing support and resources to Family Home Providers caring for individuals with developmental disabilities in their homes.

Requirements

  • Completes home visits to ensure quality of care, client satisfaction, and compliance with regulations.
  • Facilitates ISP meetings and discussions about clients' goals with clients and members of the support team.
  • Prepares written plans detailing client goals and areas where support is needed, and updates as needed.
  • Provides training and guidance to families regarding appropriate support to meet client goals and care needs.
  • Maintains charts according to specifications in home and office locations.
  • Communicates pertinent information to all members of the support team.
  • Provides on-call support to families outside of regular business hours, as needed.
  • Tracks and ensures compliance with ongoing requirements for Family Home Providers.
