Role OverviewWe are building an innovative, team-based clinic designed to deliver comprehensive, same-day care for patients with complex chronic conditions. As an Ambulatory Clinic RN Care Coordinator, you will balance hands-on patient care with meaningful clinical coordination, working primarily with patients managing chronic conditions such as Irritable Bowel Disease (IBD) and Interstitial Lung Disease (ILD).
What You Will Do
You will conduct nurse visits, review care protocols, educate patients, and administer injections or simple procedures. You will also support desk-based clinical work, including telehealth visits, telephonic nursing, pre-visit planning, and chart preparation and review.

Requirements
- High School Graduate
- Bachelor Degree Nursing
- 2 or more years of RN experience
- Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
