Job Description
Fort Health is seeking a Behavioral Health Case Manager to support practices with children, adolescents, and their families through collaborative care models. This role involves screening patients, providing patient education, tracking treatment response, and coordinating referrals. The position is part of a multidisciplinary team focusing on evidence-based mental health care in the primary care setting.

Requirements

  • Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), Licensed Mental Health Counselor (LMHC), or advanced practice psychiatric practitioner (NP)
  • Minimum 1-year of direct clinical experience working with children, adolescents and their families
  • Experience and training in delivering brief psychosocial evidence-based treatments
  • Experience with care coordination
  • Strong skills in screening for common mental health disorders and symptom assessment
  • Working knowledge of differential diagnosis of common mental health disorders
  • Strong skills in engaging parents and children
  • Developing appropriate treatment planning
  • Ability to collaborate and communicate effectively in a team setting
