Job Description
The Behavioral Health Navigator is committed to removing barriers to care by identifying critical resources for patients and helping them navigate through behavioral health and substance use disorder services in the Central Oregon region. The position operates as an extension to the primary care behavioral health team and coordinates care for patients both within and outside of the clinic.

Requirements

  • High school diploma or GED
  • Bachelor's degree in related field preferred
  • Experience with EPIC and population-based services
