Role OverviewAs a Bereavement Coordinator, you will be responsible for the management and coordination of all grief support programs and services provided by Longleaf Hospice and Palliative Care.
What You Will Do
Completing an initial assessment of patient and family to determine hospice care needs, coordinating grief counseling needs and services, and planning for Bereavement Program development.

You will be functioning independently but seeking and accepting guidance from other members of the Interdisciplinary Group or from members of the community.
Requirements
- Bachelor’s degree or equivalent theological degree from a college/university; or a qualified Social Worker; or Spiritual Coordinator/Chaplain
- Current and continued CPR certification
- At least one or two years of recent home care experience; hospice experience preferred
- Effective verbal and written communication and problem-solving skills
- Responsible valid driver’s license. Reliable, insured, automobile for making home visits required
