Role OverviewThe Case Manager is responsible for providing resources and referrals to families, conducting home visits, and advocating for families within the context of their team assignment. The incumbent is responsible for maintaining documentation for their assigned cases and completing agency assessments, case plans, and other forms as necessary.
What You Will Do
Conduct home visits, identify and articulate critical issues confronting the family, review intake summary, and determine course of action(s). Investigate resources in the community to meet the needs of clients, provide housing relocation assistance, and maintain working relationships with outside agencies.
Why It Might Be a Fit
The ideal candidate will have a Bachelor of Arts Degree in Social Work, Education or Psychology or 2-3 years work experience in the field of social work and case management. Knowledge of community resources and excellent oral, written, and interpersonal skills are required.
