Job Description
FSRI is looking for a Bilingual Community Health Worker to conduct screenings and individual needs assessments in family homes and other community venues. The CHW will engage clients, implement screening tools, and provide ongoing support and monitoring.

Requirements

  • Community Health Worker Certification
  • Bachelor degree preferred
  • Spanish bilingual required
  • Valid driver’s license, auto insurance and reliable transportation
