Role OverviewJoin our team at Merakey as a Blended Case Manager, working with up to 30 individuals in the community. You will develop and implement personalized service plans, provide case management, and offer supportive counseling. This role is part of our Monaca, PA location.
What You Will Do
Serve as the primary contact for up to 30 individuals, develop and coordinate service plans, provide case management and coordination with other agency providers, offer supportive counseling, and work with county reentry grants.

Ideal candidate will possess a high school diploma or GED, 2+ years of experience in public or private human service, and valid driver's license. Preferred qualifications include a Bachelor's degree in sociology, social welfare, psychology, gerontology, or other human services.
Requirements
- High school diploma or GED
- 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science
- 2+ years' experience in public or private human service
- 1 year of direct care experience
- Valid Driver's license
- All clearances required by OMHSAS regulations
