Role OverviewThe Blended Case Manager is responsible for developing, coordinating, and implementing services for adult participants with serious and persistent mental illness. The role includes providing resource coordination, recovery services, and symptom management using a strength-based model of case management.
What You Will Do
Provide services to 25-30 adults living with mental illness, including resource coordination, recovery services, and symptom management. Complete required paperwork, correspondence, and chart documentation.
