Role OverviewWe are seeking a community-based Blended Case Manager to join our team in Colmar, PA serving clients in Montgomery County. The Blended Case Manager is responsible for developing, coordinating, and implementing services for children and adult consumers as identified in the goal-oriented service plan.
What You Will Do
Provide case management, service planning, and coordination of services with other agency providers to meet the consumer needs and increase community stability in consumer's life. Complete comprehensive assessments, service plans, and environmental matrixes to determine level of care and frequency of contact with consumers on their caseload.

Ideal candidate will possess case management experience in mental health or behavioral health, basic computer skills, and experience with an EMR system. Must have a valid driver's license with a clean motor vehicle report.
Requirements
- Valid driver's license with clean motor vehicle report
- Basic computer skills and experience with an EMR system
- Case Management experience in mental health or behavioral health
- Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education
- Registered nurse
- A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years of experience in direct contact with mental health consumers
- A high school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989
