Role Overview

As a Blended Case Manager, you will work directly in the community to empower individuals on their journey toward stability and independence. You will build meaningful connections while assessing client needs and creating personalized care plans, link clients to vital resources, and advocate for their success.

What You Will Do

Your main day-to-day responsibilities will include promoting a rehabilitation and recovery-focused approach to case management, serving as primary case manager for assigned consumers, developing and maintaining consumer-driven service and safety plans, and collaborating with service providers.

The ideal candidate will possess a high school diploma plus 12 college credit hours in Psychology, Sociology, Social Welfare, Gerontology, or a Bachelor's degree with major course work in a related field. Requirements

  • High school diploma plus 12 college credit hours in Psychology, Sociology, Social Welfare, Gerontology
  • 2 years of behavioral health direct care experience
  • Valid driver's license, insurance, and clean driving record
