Role OverviewAs a Blended Case Manager, you will work directly in the community to empower individuals on their journey toward stability and independence. You will build meaningful connections while assessing client needs and creating personalized care plans, link clients to vital resources, and advocate for their success.
What You Will Do
Your main day-to-day responsibilities will include promoting a rehabilitation and recovery-focused approach to case management, serving as primary case manager for assigned consumers, developing and maintaining consumer-driven service and safety plans, and collaborating with service providers.
