Role OverviewThe Blended Case Manager position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan.
What You Will Do
The BCM will work and assist their assigned consumers by focusing on the following life domains: behavioral and physical healthcare needs, family functioning/independent living, educational and vocational goals, and socialization.
Why It Might Be a Fit
The ideal candidate will possess a bachelor's degree in psychology, social work, nursing, rehabilitation, counseling, or other related human services field, and experience in the human services field working with children is preferred.
Requirements

- Must have a valid driver's license and own a reliable means of transportation
