Job Description
The Department of Health and Social Services (DHSS) is seeking a Health/Human Service Case Manager I to determine initial and ongoing eligibility for needs-based benefits, provide entry-level counseling, and make referrals for benefits.

Requirements

  • Associate's degree or higher in Behavioral or Social Science or related field
  • Three years' experience in health or human services support
  • Six months' experience in making recommendations as part of a client's service plan
  • Six months' experience in using an automated information system
  • Six months' experience in narrative report writing
