Role OverviewThe Care Advocate is an integral part of the member's care team, working closely with clinicians and the clinical operations team to support the Care Delivery program. The role involves building trust and promoting health, while driving Community Health Integration and Principal Illness Navigation programs to bridge the gap between clinical care and social needs.

The Care Advocate's responsibilities include executing person-centered care plans, acting as a link between the patient and the billing practitioner, conducting person-centered assessments, identifying unmet social needs, and developing specific action plans to address them.

- Excellent written and verbal communication skills
- Outgoing personality and ability to motivate and influence different types of people
- High school diploma/GED (college degree preferred)
