The Care Coordinator will be responsible for utilizing an integrated case management model that includes assessment, planning, implementation, and monitoring.

Requirements

  • Bachelor's degree in a field related to behavioral health, social work etc.
  • Observe members, listen to concerns, and record the observations.
  • Monitor the health and safety of members, the signs, and symptoms of their mental illness, reporting any concerns to the appropriate staff in accordance with reporting policies.
