Requirements

• Work directly with children, youth, and families in community-based settings
• Coordinate care services by scheduling appointments, managing referrals, and ensuring effective communication
• Maintain accurate client records and documentation
• Collaborate with internal team members and community partners to address client needs and support care continuity
• Participate in training and professional development opportunities
• Contribute to a supportive and collaborative team environment
• Must possess a valid driver's license, reliable transportation, and be insurable under the agency's commercial policy
