Job Description
The Care Coordinator provides comprehensive strengths-based, trauma-informed case management services to individuals experiencing homelessness, mental health illness, and/or substance use disorder.

Requirements

  • Comprehensive assessments
  • Individualized service plans
  • Short-term clinical case management services
  • Motivational interviewing techniques
  • Direct crisis counseling and problem identification
  • Accompanying participants to appointments
  • Supporting participants with navigating the criminal justice and court systems
  • Identifying and connecting participants with relevant services
  • Assisting with completing applications for services
  • Providing a 'warm hand-off' to long-term services and providers
