Job Description
The Care Coordinator will provide comprehensive strengths-based, trauma-informed case management services to individuals experiencing homelessness, mental health illness, and/or substance use disorder. The role involves building relationships, providing crisis counseling, and connecting individuals to relevant services.

Requirements

  • Working knowledge of Mental Health or Behavioral Health Diagnoses
  • Harm Reduction, Housing First, Strength-based Case Management, and Motivational Interviewing skills
  • Ability to build supportive and respectful working relationships with individuals experiencing homelessness
  • Cultural sensitivity and understanding of diverse populations
  • Strong engagement skills and ability to work independently and as part of a team
