Job Description
Ennoble Care is seeking a full-time Care Coordinator, Embedded @ Partnered-Dialysis Clinics in the Tulsa, Oklahoma area. The role requires consistent on-site engagement with dialysis patients, coordinating patient care, and collaborating with providers and facility staff to ensure integrated, patient-centered care.

Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure
  • High school degree or equivalent
  • Dialysis-specific certification (CDT, CHT, CCHT, CCHT-A) highly preferred
  • Experience managing provider schedules and coordinating patient appointments within a clinical setting highly preferred
  • Minimum one (1) year of experience working directly with hemodialysis/nephrology patients in a dialysis facility highly preferred
