Job Description
Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area, with a primary focus on the Front Royal vicinity.

Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure
  • Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
  • Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
  • Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff
  • Well-developed organizational and time-management skills
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST
  • Valid, unencumbered driver’s license
  • Consistent, reliable transportation to fulfill the duties of this position
  • Ability to pass a background check and drug screen
