Job Description
Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area, with a primary focus on the Winchester vicinity. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.

Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure, such as Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), or comparable credential
  • Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
  • Ability to communicate proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
  • Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Reston, Virginia area, with a primary focus on the Winchester vicinity.
