Job Description
Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.
Requirements
Legally authorized to work in the United States of America
Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), Or comparable credential
Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person
Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long-term care setting
Well-developed organizational and time-management skills, paired with an optimistic approach and initiative-driven mindset
Strong working knowledge of healthcare terminology and care coordination principles
Proficiency with Microsoft 365, including Outlook, Word, and Excel
Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Reston, Virginia area
Valid, unencumbered driver’s license
Consistent, reliable transportation to fulfill the duties of this position
Current auto insurance, with the ability to provide proof
Ability to pass a background check and drug screen
