Job Description
This is a full time position. Care Coordinators must have open availability to work any shift (days/nights/weekends/holidays). This position is responsible for supporting and coordinating our One Call Solution, a seamless delivery of care for the region’s non-emergent transports, by working with various departments, agencies, and resources.

Requirements

  • Ability to build and establish collaborative partnerships
  • Writing, prioritization, and organizational skills
  • Ability to type 30 wpm
  • Basic computer knowledge
  • Active listening and problem-solving skills
  • Attention to detail
