Role OverviewAs a Care Coordinator, you will work collaboratively with clients, their support systems, and community providers to ensure support for overall health and wellness. You will conduct outreach activities, assessments, and care planning to help clients maintain health and wellness.
What You Will Do
Conduct outreach activities, initial and ongoing comprehensive assessments, prepare and revise care plans, maintain contact with clients, advocate for clients, and monitor and coordinate all care for clients.

Requirements
- Bachelor’s degree
- Two years’ experience in Human Services
- New York State driver’s license
