Role OverviewResponsible for providing and coordinating resident care to ensure quality care is delivered, as well as providing care team leadership. The Care Coordinator will work with the care team to ensure residents are treated with kindness and respect, and that their safety is protected.
What You Will Do
The Care Coordinator will perform rounds with physicians, communicate new orders to staff and family members, administer medications, and review and process physician orders. They will also assist with schedule coverage needs, answer resident call signals, and participate in activities as planned by Life Enrichment.
Why It Might Be a Fit
The ideal candidate will have a high school diploma/GED, completion of Care Specialist I and II curriculum within 90 days of hire, and at least 6 months of recent experience as a Care Specialist or equivalent experience. They will also have excellent communication and interpersonal skills, and be able to work with minimal supervision.

- CPR/First Aid certification
- Demonstrable knowledge and experience using Microsoft Office in a Windows environment
