Role OverviewCare Coordinators work collaboratively with clients and their support systems to ensure overall health and wellness. They conduct outreach activities, assessments, and care planning to maintain health and wellness. This role requires a collaborative team approach, with a focus on compassion, inclusion, and excellence.
What You Will Do
Conduct outreach activities, initial and ongoing comprehensive assessments, prepare and revise care plans, maintain contact with clients, advocate for clients, monitor and coordinate care, and maintain detailed record keeping.

Requirements
- Bachelor’s degree
- Two years’ experience in Human Services
- New York State driver’s license
