Role Overview

Care Coordinators work collaboratively with clients and support systems to ensure overall health and wellness. They conduct outreach activities, assessments, and care planning to maintain health and wellness. The role involves advocating for clients, coordinating care, and maintaining accurate record keeping.

What You Will Do

Conduct outreach activities, initial and ongoing comprehensive assessments, prepare and revise care plans, maintain contact with clients, advocate for clients, monitor and coordinate care, and maintain detailed record keeping.
