Role OverviewThe Care Coordinator coordinates services for women and parents of infants who were screened for or referred to Healthy Start. They provide education and support in childbirth, breastfeeding, women's health, and parenting to pregnant women and families with children aged 0-3.
What You Will Do
Responds to Healthy Start referrals, provides education and support, explains Healthy Start services, identifies risk factors, and encourages consumers to participate. Develops cooperative working relationships with primary health care providers and provides care coordination and referrals.
Why It Might Be a Fit
Ability to maintain a caseload of approximately 40+ consumers per month, attend staff meetings, and provide short-term psychosocial care if qualifications are met. Ability to provide referral information to educate and improve clients' knowledge and understanding of available community resources.
Requirements
- Graduation from an accredited four-year college with a degree in human services, social sciences, social work, nursing, health education, health planning, healthcare administration, or related field
- 2 years of public health/community development experience
- Bilingual preferred
- Must have reliable means of transportation
- CPR Certification
- First Aid Certification
- Annual Tuberculosis test
- Ability to utilize Microsoft Office Software Suite
- Ability to provide an Affidavit of Good Moral Character
- Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required
