Role OverviewThe Care Coordinator is responsible for providing and coordinating resident care to ensure quality care is delivered, as well as providing care team leadership.
What You Will Do
The Care Coordinator will perform rounds with physicians, communicate new orders to staff and family members, and assist with schedule coverage needs. They will also administer medications, review and process physician orders, and complete daily charting and documentation.

The ideal candidate will have a high school diploma/GED, successful completion of Care Specialist I and II curriculum within 90 days of hire, and at least 6 months of recent experience as a Care Specialist or equivalent experience. - CPR/First Aid certification
- Demonstrable knowledge and experience using Microsoft Office in a Windows environment
