Care Coordinator SW Sr KHC

The Care Coordination Social Worker Sr. is responsible for assessing transitional care needs, coordinating care across the continuum, and engaging with patient and family to assure care needs are met. This role serves as an expert resource for complex patient situations and as a consultant to other care team members regarding psychosocial and resource needs. The position is based at Kennestone Hospital and operates on a Day shift.

What You Will Do

Assess patients' psychosocial risk factors and resources, coordinate discharge planning, and implement timely, safe patient discharge plans. Provide psychosocial assessments, relay information about community-based services, and offer guidance on multi-system factors affecting patient/family dynamics. Mentor other social workers, participate in interdisciplinary rounds, and document all interactions in the electronic health record.

Requirements

• Master's Degree in Social Work from an accredited college or university
• LMSW in State of GA (can be waived if have LCSW)
• Minimum 2 years experience in healthcare in the acute care setting, related field or skilled care environment or community
• Excellent written and verbal communication skill
• Must possess maturity, self-confidence, objectivity, and positive attitude
• Self-directed with the ability to function well under stress, handle change, and function in a fast-paced environment
• Strong assessment, interview, organizational and problem-solving skills
• Knowledge regarding local, state and federal regulations required
• Knowledge of community and state-wide resources and programs
• Ability to work collaboratively with physicians, members of the care team, and the patient/family to assist through the continuum of care
