Job Description
Sentara Health Plans is hiring a Temporary Care Coordinator for the Northern Virginia area! The position requires travel for in-person face-to-face assessments, with the option to work remotely on days without assessments. Applicants must be located in Northern Virginia or surrounding areas.

Requirements

  • Bachelor's degree in Health & Human Services or related field
  • 1 year of Long Term Care and Health Plans experience
  • Nursing Home experience (preferred)
  • Case Management/Care Coordination experience (preferred)
  • Experience with Community Health Resources (preferred)
  • Ability to conduct assessments and develop care plans
