Job Description
Liberty Resources is seeking an Adult Health Home Care Manager for Onondaga County. The Care Manager will assist adults who are eligible for and in need of care coordination. The position is full-time, Monday through Friday, 40 hours per week during regular business hours. The position operates in a hybrid work model with required travel to conduct home and community-based visits.

Requirements

  • Bachelor's degree with 2 years of relevant experience in health or human services
  • Valid New York State driver's license and access to reliable transportation
  • Previous care coordination/case management experience working with adults living with Serious Mental Illness, developmental disabilities, substance use disorders, or chronic medical conditions
