The Care Manager plays a critical role in achieving the overall goals for coordination, integration and partnership in the delivery of care to children, adults and families with complex needs. What You Will Do

Deliver all six NYS Department of Health (NYS DOH) Health Home Core Services, including conducting NYSDOH-required assessments, developing compliant Plans of Care, coordinating medical and social service supports, and maintaining accurate documentation.

Requirements

  • Masters with one year of relevant experience
  • Bachelors of Arts or Science with two years of relevant experience
  • License as a Registered Nurse with two years of relevant experience
  • Relevant experience in care coordination, case management, social services, behavioral health, medical care navigation, or community-based services
  • CANS-NY training and certification
  • Exam score requirements: Care Managers: minimum 70%, Supervisors: minimum 80%
