What You Will Do
Deliver all six NYS Department of Health (NYS DOH) Health Home Core Services, including conducting NYSDOH-required assessments, developing compliant Plans of Care, coordinating medical and social service supports, and maintaining accurate documentation.

Requirements
- Masters with one year of relevant experience
- Bachelors of Arts or Science with two years of relevant experience
- License as a Registered Nurse with two years of relevant experience
- Relevant experience in care coordination, case management, social services, behavioral health, medical care navigation, or community-based services
- CANS-NY training and certification
- Exam score requirements: Care Managers: minimum 70%, Supervisors: minimum 80%
