Role OverviewAbound Health NJ is seeking a Care Manager to join our dedicated team in New Jersey. The Care Manager is responsible for providing support, services, advocacy, and guidance to individuals receiving New Jersey Waiver Services while upholding Abound Health NJ’s mission, vision, and core values.
What You Will Do
Key responsibilities include reviewing client service/grid notes, maintaining accurate client and staff records, conducting clinical and supervisory meetings, and participating in staff meetings and client ISP meetings.
